nurse-coordinated, digitally supported heart failure care lowers mortality by 22 percent and nearly doubles guideline therapy in India

Cluster-randomised trial in 22 Indian centres (57 percent rural): 1,507 adults with HFrEF, nurse-coordinated collaborative care with mobile health, structured counselling and real-time communication versus usual care. Days alive and out of hospital at two years: probability of survival without hospitalisation 84.0 versus 79.4 percent (p under 0.001), mortality minus 22 percent (p=0.028), guideline-directed therapy at two years 37.3 versus 22.1 percent.
