TRIUMPH-2
retatrutide yields up to 18.8 percent weight loss and a 10.8 mmHg lower systolic blood pressure in obesity with type 2 diabetes, with hypotension and dysesthesia as new signals
Presented at EASD 2026 in Milan and published simultaneously in The Lancet. TRIUMPH-2 randomised 1,152 adults with obesity or overweight (BMI 27 or higher, mean 38.2) and type 2 diabetes (mean HbA1c 7.7 percent) to once-weekly retatrutide 4, 9 or 12 mg or placebo for 80 weeks at 92 sites in eight countries. Body weight changed by -11.9, -16.8 and -18.8 percent versus -5.1 percent with placebo (treatment-regimen estimand; differences -6.9 to -13.8 points, all p below 0.0001); HbA1c fell by 1.4 to 1.6 points versus 0.2. At 12 mg, systolic blood pressure fell 10.8 mmHg, triglycerides 39.5 percent, non-HDL cholesterol 19.6 percent and hsCRP 58.3 percent (sponsor data). Gastrointestinal events dominated; hypotension (1 to 6 percent versus below 1) and dysesthesia (4 to 7 versus 1 percent) were more frequent with retatrutide; discontinuation for adverse events or death was 4, 12 and 8 percent versus 5. Seven deaths (six on retatrutide), judged unrelated. No outcome data yet; Lilly plans a US submission in Q1 2027.
