The first randomised trial in the guideline grey zone: 1,803 Korean patients with AF and a CHA2DS2-VASc score of 1 (men) or 2 (women) were randomised to apixaban or rivaroxaban versus no anticoagulation. At 24 months the composite of stroke, systemic embolism, major bleeding or cardiovascular death occurred in 0.5 percent versus 1.5 percent (HR 0.31, 95% CI 0.10 to 0.94, p=0.028); ischaemic stroke 0.1 versus 1.1 percent; major bleeding 0.3 versus 0.5 percent.
