ACASA-TAVI

a DOAC instead of aspirin after TAVI halves CT leaflet thrombosis and is as safe, with life-threatening bleeds only in the aspirin group

Lie ØH, et al.; ACASA-TAVI investigators - JAMA

Investigator-initiated head-to-head trial in three Norwegian hospitals: 360 patients aged 65 to 80 after TAVI randomised to DOAC monotherapy (apixaban, edoxaban or rivaroxaban) or aspirin monotherapy. Hypo-attenuated leaflet thickening at 12 months 17.2 versus 32.6 percent (RR 0.55, p=0.004); safety composite of VARC-3 bleeding, thromboembolism or death 7.5 versus 10.6 percent (non-inferior); life-threatening or fatal bleeding only in the aspirin arm. Hot Line 7, JAMA.
